PATIENT INTAKE
Information on booking appointments, confirming visits, registration forms, and payment policies
If you are unable to attend your scheduled appointment, please notify us at least 24 hours in advance. Appointments are in high demand, and early notice allows us to offer the time to another patient.
Failure to attend your appointment, or failure to cancel with at least 24 hours notice, will be considered a missed appointment and may result in a $75 fee.
To cancel or reschedule:
If rescheduling, please include your phone number and preferred callback time.
To help streamline your visit, new patients and returning patients with updated information are asked to complete the registration forms prior to their appointment.
Please Bring:
For Quebec residents and patients without provincial health insurance coverage, payment in full is due at the time services are rendered.
MMI accepts electronic referrals through the Ocean eReferral platform, available within most community physician EMRs.
Physicians may submit referrals by selecting Merivale Medical Imaging (MMI).
