The MMI Physician Portal provides secure, convenient access to your referred patients' imaging reports and studies. If you already have credentials, log in below.
- Diagnostic Reports — View finalized radiology reports for your referred patients as soon as they are available.
- Imaging Studies — Access full DICOM image sets from MRI, CT, ultrasound, and other modalities.
- All MMI Locations — Access results from all Merivale Medical Imaging sites across Ottawa and the Ottawa Valley.
- Secure & Private — Encrypted access governed by PHIPA and PIPEDA. Credentials are tied to your signed confidentiality agreement.

Requesting Access to the Physician Portal
New to the MMI Physician Portal? Follow the steps below to request access. A signed Confidentiality Agreement is required before credentials can be issued.
Complete the Request Form
Fill in your CPSO or CNO number, full name, clinic name, address, phone number, and fax using the request form below on this page. Incomplete submissions cannot be processed.
Download & Sign the Confidentiality Agreement
All portal users must sign the MMI Confidentiality Agreement before access can be granted. Download the agreement, sign it, and attach the signed PDF to your form submission. Patient data accessed through the portal is governed by PHIPA and PIPEDA.
Receive Your Credentials
Our team reviews all portal access requests. You will be contacted with your login credentials once your request has been approved and setup is complete.
