At MMI Medical Imaging, patient feedback is an important part of maintaining the quality and integrity of our services. If you have concerns about the care you received or the operation of one of our clinics, please complete the Patient Complaint Form.
In some cases, you may also need to provide a signed Health Information Authorization Form so that we can appropriately review and investigate your concern.
All complaints — whether submitted in writing or communicated verbally — are taken seriously and reviewed carefully. You can expect a response within 10 business days. Matters involving actual or potential harm are addressed on a priority basis.
If you are not satisfied with the outcome of your complaint, or feel it was not handled in a timely manner, you may contact the Patient Ombudsman under the Excellent Care for All Act, 2010:
